全市药学系列中、初级专业技术任职资格评审条件：申报中级任职资格的专业技术人员须通过全国职称外语B级以上考试。
关于开展2016年度全市药学系列中、初级专业技术任职资格评审工作的通知
各区县、开发区人力资源和社会保障部门，食品药品监督管理局、市场监管局，市食品药品监督管理局直属单位，市人才服务中心：
根据陕西省人力资源与社会保障厅、陕西省食品药品监督管理局《关于开展2016年度全省药学系列高、中级专业技术任职资格评审工作的通知》(陕人社函〔2016〕683号)文件规定，现就开展2016年度全市药学系列中、初级职称评审工作通知如下：
一、评审范围
全市食品药品监管系统和全市药品生产、经营企业中从事药剂、药检、制药工程、医疗器械等工作的专业技术人员申报晋升中、初级任职资格。
二、申报资格条件
事业单位专业技术人员的职称评审应以岗位设置为前提条件，各单位必须在设岗的基础上，根据岗位设置，出现岗位空缺方可申报职称晋升;具有本专业博士学位人员，符合申报条件的，可不受岗位限制申报职称资格;企业专业技术人员不受此限制。同时还要符合以下条件：
(一)思想政治条件
必须遵守国家法律、法规，有良好的职业道德和敬业精神 。
(二)年度考核
申报中级职称评审人员须在近5年的个人年度考核中获优秀等次至少1次，其他年度为合格或称职。
(三)学历资历条件
1.晋升中级职称的人员，必须具备下列条件之一：
①取得药学相关专业博士学位;
②硕士研究生毕业，取得初级资格并任现职称满2年;
③大学毕业，取得初级资格并任现职称满4年;
④大专毕业，取得初级资格并任现职称满5年;
⑤中专毕业连续从事药学工作满15年，取得初级资格并任现职称满 5年(本条仅适用山区县单位人员);
⑥取得全国执业药师资格证书的。
2.晋升初级职称的人员，必须具备下列条件之一：
①本科以上毕业取得相应学位，在本专业技术岗位工作满1年，可晋升药师资格。
②大专毕业，取得药士资格后，在本专业技术岗位工作满3年，可晋升药师资格;
③中专毕业，取得药士资格后，在本专业技术岗位工作满5年，可晋升药师资格;
④中专或大专毕业，在本专业技术岗位工作满1年，可认定药士资格。
评审条件中所称学历，是指经国家教育行政主管部门认可的药学专业或医药相关类专业(医学、护理学、生物学、化学、化工)毕业学历;所称工作年限，是指取得学历前后从事本专业工作时间的总和。
(四)外语条件：
申报中级任职资格的专业技术人员须通过全国职称外语B级以上考试。
职称外语免试按照原西安市人事局《关于完善职称外语考试有关问题的通知》(市人职发〔2007〕14号)及陕西省人社厅《关于进一步优化职称管理服务工作的通知》(陕人社办发〔2016〕26号)规定执行，申请外语免试人员，需填写《陕西省职称外语考试免试申请表》。
(五)计算机模块条件：
申报中级任职资格的专业技术人员须通过全国专业技术人员计算机应用能力考试3个模块考试。

(六)继续教育条件
按照《陕西省专业技术人员继续教育条例》和《关于做好2016年度全市药学系列专业技术人员继续教育(知识更新工程)工作的通知》(市食药监发〔2016〕241号)要求，专业技术人员每年接受继续教育的学习时间累计不少于80学时，其中公需课科目不少于24学时，专业课科目不少于56学时。
(七)为规范专业技术资格申报评审工作，防止申报中“挂靠”行为，申报对象需提供《事业单位聘用合同》。
三、报送材料及有关要求
(一)报送材料内容
1.各直属单位须提交推荐评审的报告一份，报告中要说明本单位空岗情况及被推荐人的.姓名、专业、级别、推荐理由、公开监督情况等。
2.《药学系列__级技术职称资格评审申报表》一份(附件1)
3.《西安市药学专业人员初级技术职务任职资格评审表》一份(附件2)。单位填写内容务必准确，由主管单位职改部门审核后，加盖印章。
4.《西安市药学专业人员中级技术职务任职资格评审表》(附件3)，表中基层单位和呈报单位意见栏内应写明“同意推荐×××同志晋升×××专业×××任职资格”;此外还应注明单位推荐形式、结果和公示情况并加盖单位公章。
5.身份证、最高学历证书、最高学位证书、任职资格证书、现受聘职务聘书、继续教育学分证明等;外语合格证书、计算机应用能力考试合格证(中级职称人员提供);凡符合外语计算机免试条件的相应的审批表(见附件4、5)。各种证书均上报原件和复印件各一份。
6.论文著作：申报中级职称：提交任现职或工作以来所撰写的有一定水平的专业文章2篇;能反映专业技术业绩方面的材料。
7、中级职称论文采取现场讲解和powerpoint演示文稿(演示讲述时间不超5分钟)相结合的形式。powerpoint演示文稿的内容为：个人基本情况简介、任现职以来工作业绩(近五年以来)以及论文(著)。
